PART TIME COMMUNITY SUPPORT WORKER
East Algoma

Community Living Algoma is looking to expand their Community Supports Team. Community Support workers provide supports, education and awareness of one’s community with regards to culture, recreation, spirituality, leisure and other affairs typically engaged in by all of its citizens. Each person’s supports are self-directed and reflective of his/her goals, passions, interests and strengths. In addition, supports will focus on assisting each person to develop skills and assume social roles, which promote participation in the life of the community.

DUTIES:

The Community Support Worker provides individualized support to people with an intellectual disability to achieve their personal goals during the day and/or evening. As a member of a person-centered team the Community Support Worker will:

•Support the person to participate in community based social, leisure, physical and recreational activities of their choice, develop and maintain social roles within the community.
•Promote and demonstrate the Mission, Vision, and Values of the Association within the workplace and in the community.
•Liaison with community groups and others with similar skills or interests to promote equal opportunities and participation.
•Facilitate the development of support networks for each person and promote the involvement of family, friends and community members in the person’s life.
•Utilize Personal Outcomes Measures to identify outcomes for each person supported. Implement support plans and individualized supports to achieve outcomes. Report and monitor through individual planning in direct correlation to Individual Support Agreements.
•Work effectively with families and community partners in a variety of locations.

QUALIFICATIONS:

1. Community college diploma or university degree in social services, psychology or a related field along with 2 years direct support experience in a community-based setting, or the equivalent combination of relevant education and experience.
2. Clear understanding of “Positive Approaches”.
3. Experience working with adults and their families.
4. Ability to work as part of a team.
5. Strong communication skills, both written and verbal.
6. Experience working with adults with a dual diagnosis.
7. A valid driver’s license and maintenance of an acceptable driving record is required.
8. Valid Standard First Aid Certificate (St. John Ambulance or Canadian Red Cross).

Hours of Work: up to 24 Rate of Pay: $ 23.59
